First, a little information on our Patient Portal. This Patient Portal is compliant with the Health Insurance Portability and Accountability Act (HIPPA). Yes, that is quite a mouthful! What that means to you is that the portal is encrypted and secure and you can rest assured that all of your information will remain completely confidential.
You can use the portal to request appointments, ask questions, manage your account, pay your bill online and fill out forms. Speaking of the latter, filling out forms is your next task.
Below are the 3 easy steps:
Once you have registered for the portal (the link below will take you to the "Create an Account" Page) you will be taken to the "My Client Page."
Click on the "Health Forms" link on the bottom right of the page
Finally, please fill out the "Health History Questionnaire" and the "Medical Symptoms Questionnaire" (please note the system times you out after 30 minutes to protect your privacy)
